Q. What is the data completeness threshold for 2025?
A. Data completeness is 75% of the denominator eligible patients/case with a minimum of 20 cases for each measure reported during Jan 1. - Dec. 31, 2025.
Reporting no less than 6 quality measures with one measure being a high priority/outcome. Please note: Small practices (15 clinicians or fewer) receive 3 points for not meeting data completeness.
Large practices (16 clinicians or more) receive 0 points for not meeting data completeness.
Q. What is the minimum participation to avoid the penalty for a small practice (<15 clinicians)?
A. You will need a minimum of 75 total MIPS points to avoid the penalty.
Attest to 1 high-weighted improvement activity such as reporting through DataDerm (for a minimum of 90-consecutive days between Jan. 1-Dec. 31, 2025).
6 PI measures from each of the 4 objectives for all eligible encounters between performance period: continuous 90-days between Jan. 1-Dec. 31, 2025.
CMS will continue to automatically reweight the Promoting Interoperability performance category and more heavily weight the Improvement Activities performance category for small practices.
If both Cost and Promoting Interoperability are reweighted, Quality and Improvement Activities will each be weighted at 50%.
Q. Which patient payers are eligible to report for MIPS?
A. All payers including Medicare are eligible to be reported on for MIPS.
Q. Do I have the option to report individually and as a group with DataDerm?
A. You will indicate your submission preference (individual vs. group) within the MIPS Submission Module on your Dashboard once it is accessible.
Q. Can I report traditional MIPS and MVPs through DataDerm?
A. Yes, you can report one or both pathways through DataDerm. If you choose to report both, the final score will be based on the higher of the two scores.
