To submit MIPS, DataDerm is required to validate your practice’s TIN and NPI annually. To do so, we require that you upload a submitted 1500 claims form (sample form on page 2) that is inclusive of a patient encounter from the applicable reporting year, the NPI of an active clinician, and your practices TIN.
For Group (GPRO) reporting, please submit one complete 1500 form. The NPI on the 1500 form must match any active clinician listed in the practice management section. For Individual (IND) reporting, you must submit a completed form for each eligible clinician. If you are reporting Traditional MIPS and MIPS MVP, you will need to verify the TIN for both pathways and participation types. You will not be able to access the MIPS submission module until DataDerm has verified the form and you have purchased MIPS Module access per clinician.
Example: Practice ABC has 4 Clinicians and wants to report for Traditional MIPS as Individual Clinicians and as a Group. They also want to report MIPS MVP as a Group. The practice will need to upload a submitted 1500 form for each reporting pathway.
- For Traditional MIPS IND, the practice will upload four submitted 1500 forms, one for each reporting clinician. The NPI on each form must match the NPI of the clinician being validated on the platform. The TIN on each form will be the same and match the practice TIN.
- For Traditional MIPS Group, the practice will upload one 1500 form. Only one of the clinicians NPI needs to be listed along with the TIN.
- For MIPS MVP Group, the practice will upload one 1500 form. Only one of the clinicians NPI needs to be listed along with the TIN.
Since this practice is reporting as a Group for two different pathways (MIPS and MVP), they can use the same TIN form to upload in each area within the module.

- Log in to the screen above using your AAD ID or email address and password. Click “Continue”.
- Upon logging in, you will view your main dashboard.
- To begin the NPI/TIN verification, you will need to navigate to the "MIPS Reporting" tab, then select your MIPS reporting pathway: Traditional MIPS and/or MIPS Value Pathways.
- Clinicians can report multiple pathways participation types. Clinicians who participate in multiple ways will receive the highest of their final scores
- Next, select the tab indicating how the practice will participate in MIPS: Group, Individual, or Subgroup* (*only available for MVP reporting). Note: If you are a practice with less than two providers, you will only have the choice of reporting as an Individual.
- For all participation types, the TIN listed in Box 25 must match the TIN listed in the platform.
- For GPRO reporting, please submit one completed 1500 form. Please note that the NPI on the 1500 form (24. J) must match an active/enabled clinician listed in the practice management section.
- For IND reporting, you must submit a completed form for each clinician who is eligible to report. The NPI on the 1500 form (24. J) must match the clinician to whom the file was uploaded.
- To upload the PDF, navigate to the TIN Validation column, then select the “Upload PDF” button.
- The browser will open a new window to the folders on your computer. Please upload the PDF of the submitted claim forms by selecting the file.
- Next, click “Open,” which will upload the file. Once successfully uploaded, the TIN validation will display “Awaiting Validation”.
- If you uploaded the wrong document, please open a support ticket with DataDerm for it to be deleted. Once deleted, you can reupload by selecting the “Upload PDF” button.

- DataDerm will confirm via a support ticket to the email that uploaded the form if the TIN/NPI was validated or rejected. If the form is accepted, we will provide you next steps. If the form is rejected, we will provide reasoning in the ticket, and you will need to upload another 1500 claims form.
- Once the TIN/NPI is verified, clinicians can purchase their MIPS reporting module for each NPI that is included in reporting under their own AAD Member ID.
